How to apply for insurance:
1. Print the
application form (PDF Format), complete, sign and date it.
2. Send the form with your premium check and the following forms:
- For horses owned at least one year & insured for $50,000 or less, print, complete, sign and return the
Statement of Condition (PDF Format) with the application form.
- For newly purchased horses and/or those insured for more than $50,000 print the
Vet Certificate (PDF Format), ask your vet to complete and return the certificate with the form.
- For Loss of Use Coverage (Hunters, Jumpers & Dressage only), please include a pre-purchase exam and/or loss of use exam.
- If you are leasing a horse from another person, the applicant must be the owner. You will be listed as "lessee" at question 4.
